Amateur TITLE Bout Agreement

Amateur Mixed Martial Artists, Managers, Trainers:

This bout agreement is to serve as confirmation that an amateur Mixed Martial Artist, herein named, has agreed to compete in an Event for Cagezilla Fighting Championship ("CageZilla") on the date specified.  It is clearly understood by the amateur Mixed Martial Artist, his trainer, team and/or manager that the participation in this Event can be dangerous and he/she is entering into this Agreement freely and accepts the risk involved.  Mixed Martial Artist warrants that he/she is free to enter into this Agreement to exhibit the skills of a mixed martial arts fighter and to hold promoter, owners and ALL associated parties with the event harmless for any and all injuries he/she may sustain in the course of this or any other bout with Cagezilla.  Mixed Martial Artist is self-insured and acting as an independent contractor subject to the laws, rules and regulations regarding an independent contractor is required to follow in the State in which he/she is performing. 

This Agreement is entered into this Agreement Date , between Cagezilla FC and   , an amateur Mixed Martial Artist, who lives in the city of    and the state of    is for a MMA bout of three (3) rounds (five (5) minutes per round) at a weight of   , under Unified Rules, on   , against (opponent). 

This bout will take place at The Salisbury Center, located at 8890 Mathis Ave, Manassas, VA 20110 in accordance with the rules and regulations as set forth and determined by the Virginia Boxing, Martial Arts and Wrestling Commission.  There will be NO COMPENSATION for this bout.  Mixed Martial Artist agrees to abide by all of the rules of the state of jurisdiction and perform to the best of his/her ability.  Although CageZilla has medical coverage in case of catastrophic injury, Mixed Martial Artists must have their own medical insurance and CageZilla insurance shall be subordinate to Mixed Martial Artist's insurance.

Cagezilla reserves all the rights to the ownership of this match in its entirety, including, but not limited to; TV, radio, billboards, photos and all other ancillary rights.

In the event of a NO SHOW or other violations of this Agreement, Promoter reserves the rights to file a formal complaint with the Virginia Boxing, Martial Arts and Wrestling Commission.  Mixed Martial Artists must weigh-in either the night prior or make arrangements with CageZilla to weigh-in at another time prior to their bout at The Salisbury Center, 8890 Mathis Ave, Manassas, VA 20110 unless otherwise notified in advance.

Mixed Martial Artists may not participate in another athletic contest (boxing, MMA, BJJ, etc.) 45 days prior to the date of the scheduled bout in this agreement without approval from Cagezilla.

Mixed Martial Artist agrees to all the terms and conditions stated above.
