In order to fight for CageZilla Fighting Championship or Thai Championship Boxing in VA all amateur fighters must complete the following items prior to weigh-ins, which are held 24 hours before each event.  Click on each item to be linked to the appropriate form:

All fighters must register for the event or events in which they would like to participate by completing the appropriate registration form below.  Once our match making team receives their registration someone from the team will follow up with the fighter, if they’re an independent fighter, or the fighter’s coach if they are affiliated with a team, school or gym to get permission for them to fight and to coordinate potential matches.

Fighter Membership Registration

Once a fighter has been matched with an opponent they must complete and sign a Pro Fight Agreement.  These Contracts are created on a case by case basis and negotiated between the promotion and the fighter and/or his coach or management.

Once Bout Agreements have been completed fighters must complete the Liability Waiver form, the Media Release/Waiver form and submit two pictures of themselves for the digital fight card and marketing of their fight.  Pictures should be of fighters in a fight stance and fighters with their arms folded in front of them.  For best results fighters should be in front of a solid background that is in contrast to the color of their clothing, hair and skin.  These pictures can be taken on a smartphone after practice and do not need to be professionally done but the better the lighting and higher the resolution the better the marketing materials will be.

All fighters, pro or amateur must have a medical clearance from a doctor within 180 days of the event they compete on.  They may use EITHER of the following forms for your personal doctor or another doctor to complete and sign or they may use their own forms.  EITHER form is adequate as well as the standard sports physical forms most doctors and urgent care facilities have on-hand in their offices.  These can be obtained from your personal doctor, which sometimes takes over a week for you to receive, or you can usually obtain a signed form on the same day you are examined for a sports physical at an Urgent Care, Patient First or Minute Clinic.  Minute Clinic is usually the cheapest of these options.

Fighter Physical/Medical Clearance Forms

All fighters, pro or amateur must have a blood work from an accredited lab within 180 days of the event they compete on.  The lab will usually have their own form but the results much show your name, the date you were tested, the date the test was done, and that you are NEGATIVE or NON-REACTIVE for HIV, HEPATITIS B and HEPATITIS C.  These test can be obtained from your personal doctor, which sometimes takes over two weeks for you to receive, or you can usually schedule and obtain a test on the same business day or day after you call with results within 24-48 business hours afterward from one of the following websites, requestatest.com or advancedsportslabs.com.  We are not affiliated with either and get no kickback from them.  They specialize in these tests for fighters and do a good job with aligning fighters with tests and results in a timely manner.
